Aboriginal Health Promotion and Chronic Care Program

This program is to improve health outcomes for Aboriginal and Torres Strait Islander people in the Swan Hill area including those with chronic disease or at risk of developing chronic disease, through health promotion, prevention, early intervention, and ongoing care.
